Unlock Cheat Mode

While at the main menu, press L, L, R, R, X, X, Y, Y, B, B.  When you start a game and pause it, there will be a "Cheats" option in Option Menu.

Unlock Extreme Mode

Beat the game on Hard Difficulty.
